We will offer both remote and in-person enrollment options for the 2022-2023 school year. Both options are detailed below.
In-Person Enrollment
Now that the 2022-2023 School Year is underway, if you have a student within Shoesmith's attendance boundary to enroll for the current school year, please call Shoesmith main office to make an enrollment appointment with our school clerk, Ms. Henderson at (773) 535-1765.
Note: For families receiving STLS supports, please call (773) 535-1765 to schedule an in-person enrollment appointment.
To ensure the health and safety of our families and staff members, please adhere to the following guidelines when enrolling your child:
Kindergarten through Sixth Grade Required Enrollment Documents
1. Original Birth Certificate
2. Transfer document from sending school
3. Medical Forms:

Remote Enrollment
Due to the Covid-19 health pandemic, Shoesmith will offer a remote enrollment option for incoming families enrolling their student(s) for the 2022-2023 school year. The procedure will be as follows:
1. Complete the Shoesmith's Electronic Enrollment Submission Form. This Google Form will be used to upload and submit all required documentation listed below.
2. Submit copies of the required enrollment documents in the electronic form above OR arrange to drop off copies of the documents to Shoesmith - copies of the required forms can be found above.
3. Once the Electronic Enrollment Form and the required documents have been received, Shoesmith's clerk will call to complete the registration process over the phone.

To ensure the health and safety of our families and staff members, please adhere to the following guidelines when enrolling your child:
- Only one parent/guardian per family may enter the building to enroll your student(s) - please do not bring student(s) with you.
- You must wear a face mask that fully covers your nose and mouth to enter the building. Masks must be worn for the entire duration of the enrollment process.
- You must enter and exit through Shoesmith's main entry (Door #5).
- You must wash your hands or use hand sanitizer provided by the school when you enter and exit the building.
- You may not enter the building under the following circumstances:
- You are ill.
- You have been ill with COVID-19 and your symptoms first appeared less than 10 days ago.
- You have been ill with COVID-19 and have had a fever or respiratory symptoms (e.g., cough, shortness of breath) within the last 72 hours.
- You are awaiting COVID-19 test results.
- You have been in close contact with someone who has tested positive for COVID-19 in the last 14 calendar days.
- Enrollment areas will be set up to ensure social-distancing practices are in place (i.e. 6ft apart, no shared materials, materials disinfected after use, etc.).
- Restrooms will not be available to the public.
- Please have all required enrollment documents with you to ensure a quick registration process. See the below list for required registration documents.

- Food Substitution Form (For students with food allergies only)
- Lease, deed, or mortgage AND...
- Diver's License or State ID with matching address
- TWO of the following documents dated within the last 60 days:
- Utility bill, home or renters insurance document, or voter's registration card
- **If residing with someone, you must show notarized letter, I.D., lease/mortgage and utility bill of person you are residing with.
